Label each final dosage container.

Improperly or unlabeled CSPs have been a source of patient injury and death. It cannot be assumed that anyone other than you will know what is in a final container. When confronted with two unlabeled 10mL syringes of clear fluid, you can’t be sure if you have the right drug. Label it when it is mixed or if you find something unlabeled, throw it out. It may save a life!
